FALL TACKLE FOOTBALL AND CHEER PROGRAM ONLY:
Football required forms: Medical Release, Parent Code of Conduct , Player Code of Conduct, Concussion form, Registration Form, and top of equipment form
Cheer required forms: Medical Release, Parent Code of Conduct , Player Code of Conduct, Registration Form, and top of equipment form.
Birth Certificates are also required on Registered. Players will not be able to practice if all forms and documents are not turned in by this date.
